The Committee on Health and Human Resources moved to amend the bill on page 2, line 15, after the word, "order" by inserting a comma and the following: "if the Department of Health and Human Resources is the legal guardian and elects to make the anatomical gift, the most immediate relative of the donor must be notified at the time of consent the donation is signed, if able to be located.  The medical record shall reflect documentation of the notification or attempted notification.  The same effort should be made regarding notification of a family member in the case of an incarcerated individual. "
